The Vial of Life provides important health information in the event of a medical emergency. Participants in the program complete the Vial of Life form, which includes emergency contacts, prescription information and significant medical conditions. This form goes into a pill bottle that is placed inside the refrigerator. If you live in Addison, Carrollton, Coppell or Farmers Branch, your city's EMS department is notified when you sign up as a Vial of Life participant. The paramedics will then know to look in your refrigerator for important medical information. Outside these areas, contact your local Emergency Response System for more information. Pick up your Vial of Life Startup Kit at Senior Adult Services, your community fire station, or a community health fair. Remember to update your information as your medical conditions change.
