Vial of Life

......................
The Vial of Life provides important health information in the event of a medical emergency. Participants in the program complete a form which includes emergency contacts, prescription information and significant medical conditions. This form goes into a pill bottle that is placed inside the refrigerator. Your city's EMS department is notified when you sign up as a Vial of Life participant and first responders will then know to look in your refrigerator for important medical information when called to your home.
Pick up your Vial of life startup kit at Senior Adult Services, your community fire station, or a community health fair. Remember to update your information as your medical conditions change. If you are already enrolled, click here to print an update form. To request more information on the Vial of Life program please call us or click here for contact information.
Senior Adult Services provides the Vial of Life to seniors living in Addison, Carrollton, Coppell and Farmers Branch. Outside these areas, contact your local Emergency Response System for more information.
